A Day In The Life Of A Care Coordinator

Our Care Coordinators are the most important part - the heartbeat - of the Seniorcorp team. They are empowered to be your advocate in making sure that every part of your plan is accomplished.

Each Care Coordinator is selectively recruited and must have the training and compassion to serve seniors and their families. Often times our Care Coordinators are considered part of the family.

During an average day, our Care Coordinators do many things. On a daily basis they are calling or visiting clients to see how they are doing. They accompany clients on doctor visits. Care Coordinators check-up on visiting nurses aides, and make sure medications are being dispensed effectively.


Care Coordinators make suggestions on home modifications or equipment, such as shower chairs and gel cushions (to stop bedsores,) and facilitate purchases. She ensures delivery services are on time.




 


 

They receive phone calls from family members and respond with helpful advice. Perhaps there is a question regarding health insurance: the Care Coordinator immediately has a Seniorcorp insurance advocate call the insurance company for the family.

She may surprise her client with flowers. At Christmas time, the Care Coordinator may help decorate the home. Many days, she just holds the client’s hand and listens.

At Seniorcorp, we believe that our most important task is to hear and respond to the needs of our clients. We turn these needs and our evaluations into a Plan of Care that our clients and their family are well satisfied with. Your personal Care Coordinator implements this Plan of Care with great care and satisfaction.
